You must be:
- A Pennsylvania CHW
- Pursuing career advancement/enrichment (Request must clearly demonstrate purpose of career advancement and/or career enrichment)
- Unable to attend without the financial support of the PA CHW Collaborative (this application does not serve as a substitute for financial support from your employer)
This application is for reimbursement purposes. You will be required to submit all original receipts within 30 days of purchase. Please submit your request as early as possible – preferably at least 30 days before requested event date. Each application is subject to review by the PACHW Board of Directors.
Funding is limited. Submission of this application does not guarantee funding.
