Rebecca Miller is the Network Director, Outpatient Care Management for St. Luke’s
University Health Network. She has over 26 years of nursing experience, the last 10 years in Population Health, Value-Based Care, and Care Management. Rebecca has been in her current role for 6 ½ years. In 2020, SLUHN decided to move from a predominantly all RN Care Management model to a multidisciplinary model which included Community Health Workers. Our CHWs are assigned to a team with an RN and SW care manager and cover a pod of primary care practices. They mainly work with the Medicare/Medicaid vulnerable populations to connect the patient’s to community resources leading to better access to health care. She has been active with the Collaborative and participated in Rally Day and various presentations.
